Steel City Softball League is committed to fair, safe, and inclusive play. The Player Inclusion Policy (PIP) outlines how players who may require individualized evaluation can participate in league play.
This page is for licensed medical professionals who have been asked to submit a written evaluation on a player's behalf. If you are a player or family member, please contact the league directly — do not submit this form yourself.

Privacy & Data Retention
- Submissions are stored in a private Microsoft 365 SharePoint list visible only to the Secretary and Commissioners of the Steel City Softball League Executive Board.
- Microsoft 365 is HIPAA-compliant and operates under a Business Associate Agreement (BAA) with Steel City Softball League. Data is encrypted at rest (AES‑256) and in transit (TLS 1.2+), and Microsoft is independently audited under SOC 1, SOC 2, ISO 27001, and FedRAMP.
- While the information you submit is not classified as Protected Health Information (PHI) under HIPAA, SCSL voluntarily handles it using HIPAA-aligned safeguards as a best‑practice baseline.
- All submissions are automatically deleted at the end of each calendar year (December 31). A new evaluation is required each season.
- SCSL does not sell, share, or repurpose this information. It is used solely for league inclusion decisions.
