Procedures Filing an Insurance Claim

All individual playing members of the SCSL are covered by the League’s supplemental insurance except as notified.

  1. As soon as it is possible, the injured player should inform their manager of the injury.
  2. The manager is responsible for contacting a league executive board member with the facts of the injury/injured, including:
    1. Day/time of injury
    2. Name of person injured
    3. Daytime telephone number for which the injured person can be reached
    4. Type of injury sustained
  3. The members of the executive board entrusted with this information will pass the information to the Treasurer of the league who will be responsible for preparing the insurance claim. Please be aware that the insurance carried by the SCSL is only supplemental. It will only pay after your personal insurance has been utilized and a total of $250.00 deductable has been paid by the injured person. If the injured person has no personal insurance, the supplemental insurance acts as a true insurance policy with a $250.00 deductable.
  4. The league treasurer will contact the injured player and will provide them with a form which must be completed by the injured player. It is the responsibility of the injured player to obtain this form.
  5. The form provided must be returned to the league treasurer within two (2) weeks. The league treasurer will then send this form for signature to the Western PA Softball Commissioner.
  6. During this interim time period, the injured player must provide copies of all bills resulting from the said injury and pass to the league treasurer. Once the approved claim form has been received from the Western PA Softball Commissioner, this form, copies of the bills, and a clarification letter will be submitted to the league insurance agent. A copy of all transmittals will also be provided to the injured player.
  7. The league insurance agent (currently Bollinger Insurance) will be instructed to hold any necessary discussions with the injured player at this point. It will be the responsibility of the injured player to contact Bollinger if conflict and/or issues arise. The league treasurer is open for consultation if necessary.
